Please note: Emergency Medical Reports are available for individual student downloads only. The report cannot be downloaded in bulk for all students.
Key Features
The report provides a clear, printable PDF containing important student information, including:
- Student name and identification details
- Date of birth and age
- Year Level / Home Group
- Student photo
- Parent/guardian contact information
Medical Information
The report includes critical medical and health information, such as:
- Anaphylaxis, Asthma, Allergies, Epilepsy, Food intolerances…
- Other health conditions
- Medicare information
- Private health insurance details
- Ambulance membership

Accessing the Emergency Medical Report
Schools can access the report from either of the following locations:
Option 1:
Apps > Wellbeing / Medical / First Aid > Medical > View Medical > Medical Quick Actions > Emergency Medical Report
Option 2:
Apps > Users > Students > Medical > Quick Actions > Emergency Medical Report
